We have a Lung (Pulmonary) Rehabilitation program that provides information on lung function by specialist health professionals. The program can help you achieve your personal goals and improve your quality of life.
You can take part in exercise sessions run by physiotherapists that can help you to manage daily movement and breathlessness.

Our service can assess your needs and provide specific information to help you manage your lung condition. Your nurse will liaise with your doctor and medical specialist to ensure your care is coordinated and meets your needs. Our staff can provide you with specific and practical information to help you manage your lung condition including:
- Recognising and monitoring symptoms
- Supporting you with a monitoring app for your condition on your mobile phone
- Daily management
- Managing your medications and equipment
- Preventing and managing flair ups – Action plan support

Acute (sudden) Lung conditions
Do you have Asthma &/or have experienced an event during the thunderstorm asthma period September to November?
It is extremely important that you work with your GP and /or lung specialist to manage and control your condition. They will provide you with an action plan that is regularly reviewed and ensures you understand the steps that are required when your asthma is “flaring up”.
Your medications and inhalers are of vital importance, and you should always carry these with you.
If you would like further support with your asthma management, please contact your GP/ Specialist and arrange a review.

Acute (sudden) Heart (cardiac) conditions
If you have recently experienced hospitalisation for a heart attack, stent, angiogram, or insertion of a pacemaker you may be referred through Community Care intake service 1800 654 324 or you can refer yourself, to the acute heart (cardiac) rehabilitation program.
Our service will assess your needs and provide specific information to help you understand your heart event to ensure optimal health going forward. Your nurse will liaise with your doctor and medical specialist to ensure your care is coordinated and meets your needs. Our staff can provide you with specific and practical information to help you understand and manage your heart event including:
Factors impacting a healthy heart.
- Poor diet
- Overweight
- Non-compliance with medications
- Smoking
- Genetic background
- Ethnicity
- Lack of exercise

The Acute Cardiac Rehabilitation Program helps you:

Cardiac rehabilitation usually runs for 8 to 10 weeks. It often starts in hospital and continues when you go home. Programs can be run in different ways and places, for example: face-to-face, over the telephone, in a group or one-on-one and also in your home.
