Heart (Cardiac) Care Services

If you are living with heart disease we can help you return to an active and satisfying life and work with you to prevent any recurrence of cardiac events.

Having a heart problem can often be a shock and you are given a lot of information.

Cardiac rehabilitation services can help you during the hospital admission and then after discharge home.

You will be provided with support education and information to assist you through your hospital stay.

Patients are assessed by the Cardiac Rehabilitation team of Nurses and Exercise Physiologist and each client is provided with an individualised education and exercise program, which is tailored to provide support and promote recovery.

You will have access to a multidisciplinary team which includes nursing, physiotherapy, dietetics, occupational therapy, social work and pharmacy. Our team will help you identify the goals you want to achieve and support you.

Cardiac rehabilitation usually runs for 8 to 10 weeks. It often starts in hospital and continues when you go home. Programs can be run in different ways and places, for example: face-to-face, over the telephone, in a group or one-on-one and also in your home.
